For good and valuable consideration, receipt of which is hereby acknowledged, I hereby grant to Consumers Credit Union, its successors and assigns, the right to photograph me and record my voice and image, and the right to use such photographs and recordings, including reproductions and composites thereof or likeness based thereon, with or without my name or with a fictitious name, in any manner without limitation as to change, duration, frequency or territory in connection with the advertising and public relations of the Consumers Credit Union, its affiliated corporations and organizations, their successors and assigns, in any or all means of media, and the right to copyright same.

I waive all right of inspection, approval and compensation of any kind, and release Consumers Credit Union, its affiliates and their successors and assigns from any and all liability arising out of the exercise of the rights hereby granted.
