Please follow these steps to complete the Verification of Deposit (VOD) process:

  1. Download and complete the VOD Request Form and ensure that it is signed by the Consumers member. Members can also request and print a VOD from their Online Banking account.
  2. Fax the VOD Request Form to 269.743.5124.
  3. A Verification of Deposit will be returned to you by fax within two business days.
